Your dental record
When you become a patient we collect your name and contact details, your health card, your health history and the medicines you take, your insurance details where you ask us to bill a plan, and the clinical records, X-rays and photographs made while treating you.
We use them to provide dental care, to bill your plan or the Canadian Dental Care Plan where you have asked us to, and to meet the record-keeping rules set by the Royal College of Dental Surgeons of Ontario. We hold them under Ontario's Personal Health Information Protection Act, 2004.
The dentist and the staff involved in your care see your record. An insurer sees only what a claim requires. Another dentist or specialist sees it if you are referred, and only what the referral needs. We do not sell information to anyone, and we do not use it for advertising.
Dental records are kept for at least ten years after your last visit, or for a child, ten years after their eighteenth birthday, and are then destroyed securely.
This website
The appointment form and the referral form send what you type in them to the practice as an email. Nothing is stored on the website, and there is no patient portal or database behind it. Please keep medical details for the appointment itself: the form only needs enough to phone you back.
The site sets no cookies, runs no analytics, and loads no advertising or social-media trackers. Its only outside request is for the typeface, from Google Fonts. Cloudflare, which serves the site, keeps short-lived security logs that include IP addresses, to block attacks and abuse.
Email is not a secure channel. Anything you send us by email, or through the forms, travels the way ordinary email does — which is why records and X-rays are arranged by phone instead.
Your rights
You may ask to see your record, ask us to correct it, ask for a copy, or ask us to send it to another dentist. Write to info@allcaredental.ca or phone 647.479.3151. A copying fee may apply where the law allows one, and we tell you the amount first.
If you are not satisfied with how we have handled your information, you can complain to the Information and Privacy Commissioner of Ontario: ipc.on.ca, 1-800-387-0073.
